When I start feeling the usual aches I've become used to, I notice that I start moving around to correct it with this instead of sitting on my feet or propping my legs up like I used to. Sometimes I'll just lay completely horizontal on it for a little while until my back feels better, and it's great. In addition, when I am exercising, I've been able to naturally and intuitively target portions of my abs that before felt a lot more cumbersome because of the hard floor factor.
The reason I like the Gaiam one in particular is because it's a lot sturdier than some that I've used. The material is really thick, and the pump works quite well despite the feeling I got when I first started pumping. There are 3 little white stakes included to seal the air pocket.
You'll get a heavy chemical-laden smell out of the box that will go away in a few days. That's with any of these. To expedite it, it's important to soak it with soap and hand wash it before you inflate it. These things are made in plastic factories, not in daisy fields. I've sent things back before (like weights) that were unbearably harmful and smelly, but this is reasonable and worth the quality trade-off.
Overall, I think this ball is going to last, and like I said, it's great for exercise, posture, movement, and sitting. I don't miss the so-called "back support" of a chair now that I've been using this. It never existed!
If you are using this as your desk chair: BUY THE BIG 75CM ONE! I read about the sizing, and since I am just over 5' tall, I bought a smaller ball. I think it would have been a good size to use while working out, but it was way too short for my desk. So buy a 75cm one if you are using it as a desk chair.

When I first got it, I did have to re-inflate it a few times, probably because it was being stretched out for the first time. Now it rarely needs any reinflating, and if it does it's just a few pumps.
The hand-pump included works just fine too. It's sturdy and made with hard plastic.
I haven't tried the workout CD it came with, since I mostly just use it for sitting on, so that did not factor into my review.
It definitely helps me stay awake at work, but I don't use it as often as planned. I planned on using it as a full time chair replacement, but find that most days I don't have the energy. I end up sitting in weird positions and prop myself on my desk and my back starts to hurt until I switch to my chair. Some days I'll use it all day, but most days its a pretty even split between this, standing and a chair. [keep your chair handy until you see how often you use this]
I am 5'8'', but I got one size up (75cm) since a few reviews said the 65 felt too short for a chair. The 75 is perfect height and I think the 65 would feel too small.

The pump kind of sucks, and was a little bent when I got it. It took a long time to blow up the ball.
A few hours in the material stretched, I had to pry out the plug (really hard to do) and blow it up more, and a few days later I had to do the same thing. A couple weeks in it could use a bit more air, but I've been too lazy so I just got used to it.
The resistance band is useless and I haven't looked at the dvd yet.
Overall this is a great ball with a crappy pump. If it seems like it lost pressure, it probably just stretched a bit and needs more air. I wasn't surprised or concerned by this, but some people might be.
